Directors and Officers Insurance Proposal Form
If the business in the last reporting period did not make a profit and have a positive net worth (this includes startup businesses), you will be required to complete the below directors and officers insurance proposal form to proceed with your application. Once you have completed and signed the proposal form you will be emailed a copy for your records. Before we solicit quotations, we shall review the information submitted and offer feedback where necessary about answers that may give insurers cause for concern.
